This webinar, offered by the United States Health Resources and Services Administration (HRSA), provides learners with an overview of how to use the Uniform Data System (UDS) mapper to identify prevalent social determinants of health in your service area. This is important for assessing community health needs and identifying a population for the COPC project.
Rationale: This recorded lecture provides fundamental information for how to utilize a geographic information system (GIS) to identify and visualize social determinants of health and other information on the communities you treat. This tool can be used to 1. explore in PGY-1 and 2. identify a population or target social determinant of health/population health issue to address in your COPC project in PGY-2.Other resources for the UDS Mapper can be found in the link below b. https://udsmapper.org/tutorials/Other GIS programs are available, such as the Healthlandscape program (see link below)c. https://healthlandscape.org/There are a number of data sources for population health and social determinants of health; however, the ones presented in this module are from trusted institutions with a greater likelihood of offering reliable data. Other data are welcome, so long as they are from trusted sources. Residents at TWCGME are expected to complete a Community Oriented Primary Care (COPC) project, which addresses health disparities and social determinants of health in TWCGME's service area. This module offers tools that can be applied when approaching the COPC project requirement, as well as learners' personal projects.
