Certified Lab Instructor (CLI) Course has been scheduled:

Program Coordinator: David Brenner, CIC, RF
Required: Monday, August 19, 2024 (8am-5pm); Tuesday, August 20, 2024 (8am-5pm); Saturday, September 7, 2024 (8am-3pm); and Sunday, September 8, 2024 (8am-3pm). All classes will be held at LaGuardia Community College. Students must attend every class, be punctual, and remain until the class is dismissed.

• There is a $125 course fee for each candidate. Payments must be made in the form of a Money Order to the Regional EMS Council of NYC. Credit cards are also accepted. Call 212-870-2301.
• Once a candidate is accepted into the course, no refunds will be made. Entry into this course does not guarantee passing.
• Candidates must show skill mastery as part of the CLI course. Course Sponsors should only recommend candidates who can fulfill this requirement.
• Students should purchase ‘Foundations of Education: An EMS Approach’ written by the by National Association of EMS Educators' prior to start of class.
The candidate application package must contain electronic copies of:
1. Letter of recommendation on official letterhead from the Course Sponsor Administrator of the Course Sponsorship that will provide the instructor internship. Please use the format provided in the link: course_sponsor_letter_tplate.docx (live.com)
2. NYS DOH form 65: Application for EMS Certification (https://www.health.ny.gov/forms/doh-65.pdf [health.ny.gov]) – if this completed form is not received, candidate will be excluded from class list.)
3. Candidate’s current EMT/EMT-P card
4. Proof of active practice from an EMS Agency on official letterhead and signed by the chief officers. An agency representative must verify that the candidate's name is on the agency's roster and patient care reports. See below:
NYS DOH REQUIRES that candidate must be actively providing on-going, direct, hands-on, pre-hospital patient care with a NYS Certified EMS agency, at the EMT level or higher for at least one year within the last three years.
